1. Monthly Pass Payment Rate: RM 70.00 per month with a minimum payment rate of 1 month (RM 70.00)

2. A deposit payment of RM 70.00 (100% refundable) will be charged upon registration.
3. Monthly Pass renewal is 7 days before the expiry date.
4. The time of use is from 12.00 am to 11.59 pm.
5. Comply with the rules set by the parking management company (Centrio Parking).
6. One Pass is reserved for one car only according to the registered vehicle number.
7. This pass is valid for use in the open parking area next to Pantai Hospital only. Please obey the Parking Signs in the area / road concerned.
8. If the Monthly Pass holder wants to change the vehicle number, it is mandatory to fill in the existing Monthly Pass Application Form again and a replacement charge of RM2.00 will be charged with the same expiry date.
9. Monthly Pass owners are not allowed to conduct business activities in the parking lot. In case of breach of conditions, the Monthly Pass will be canceled without refund.
10. The vehicle owner must include an identification card and a photo of the vehicle when registering.
11. The Monthly Pass is only validly registered if the vehicle is categorized as a moving vehicle only.
12. Please make the payment via online payment in the bank account as stated below.

2. Pembayaran deposit sebanyak RM 70.00 (100% boleh dikembalikan) akan dicaj semasa pendaftaran.
3. Pembaharuan Pas Bulanan adalah 7 hari sebelum tarikh tamat.
4. Masa penggunaan adalah dari jam 12.00 pagi hingga 11.59 malam.
5. Mematuhi peraturan yang ditetapkan oleh syarikat pengurusan parkir (Centrio Parking).
6. Satu Pas dikhaskan untuk sebuah kereta sahaja mengikut nombor kenderaan yang didaftarkan.
7. Pas ini sah digunakan di kawasan tempat letak kenderaan terbuka sebelah Hospital Pantai sahaja. Sila patuhi Papan Tanda Tempat Letak Kereta dikawasan / jalan berkenaan.
8. Sekiranya pemegang Pas Bulanan ingin menukar nombor kenderaan, diwajibkan mengisi semula Borang Permohonan Pas Bulanan sediada dan caj gantian RM2.00 akan dikenakan dengan tempoh tamat yang sama.
9. Pemilik Pas Bulanan tidak dibenarkan menjalankan aktiviti perniagaan di dalam petak letak kereta. Sekiranya berlaku pelanggaran syarat, Pas Bulanan akan dibatalkan tanpa wang pulangan.
10. Pemilik kenderaan perlu sertakan kad pengenalan dan gambar kenderaan semasa melakukan pendaftaran.
11. Pas Bulanan hanya sah didaftar jika kenderaan dikategorikan sebagai kenderaan bergerak sahaja.
12. Sila buat pembayaran melaluis pebankan di talian di akaun bank seperti dinyatakn di bawah.
